Elder Care NavigatorTM sits outside the health and aged care service delivery system and promotes self-determination and autonomy for older people and their families. Our service is relationship-based and designed to make it easy to make decisions, put in place services and supports, and make sure the important things are attended to. Through regular and planned visits, our professional Navigators assist with the provision of services for Home Care, Personal Care, Lifestyle and Transitions.

HOME CARE – Identifying ways to assist with household tasks that are required to maintain the home and gardens, shopping, and meals

LIFESTYLE – Ensuring family, social and leisure activities occur

TRANSITIONS – Providing support and advice when needs change, and assisting with moves to new living, care or hospital facilities.
